Patient Info and Forms
A little preparation can help your first appointment run smoothly. Before your visit, please complete any new patient forms provided by our team and bring the information requested by the clinic.
Your first visit gives your physical therapist an opportunity to listen to your concerns, learn what you want to get back to doing, and perform a thorough evaluation. We use that information to determine what is limiting you and develop the right treatment plan for your needs.
Depending on your situation, it can be helpful to have:
- Completed patient forms
- Your photo ID
- Your insurance card, if using insurance
- A list of current medications
- Relevant physician information or referral paperwork, when applicable
- Any questions you want to discuss with your physical therapist

Insurance Information
Insurance can be confusing, and we want you to have clear information before beginning care. Meier & Marsh Physical Therapy works with patients to help them understand the insurance and payment information related to physical therapy.
Coverage varies by insurance plan. Your deductible, copay, coinsurance, visit limits, referral requirements, and other plan rules can affect what you pay for care.

Direct Access
You may be able to see a physical therapist without first getting a referral from a physician. This is known as direct access.
Direct access can make it easier to have pain, an injury, or a movement problem evaluated without adding another appointment before physical therapy. Our specialists can assess your condition, determine whether physical therapy is appropriate, and begin treatment within the requirements of state law.
There are situations where a physician referral or additional medical evaluation may still be needed. Insurance plans can also have their own requirements for coverage.
